Patient Info & Patient History PDF Forms
Here is another, related, pet peeve of mine, medical forms. Fortunately, I don’t have to deal with them very often. Recently I did see a new doctor and was pleased to be told when setting up the appointment that I could go to their website, download their forms, fill them out and thus save time in the office. I went to their site and downloaded the forms and found what I expected, non-fillable PDF forms. So I made them fillable (thus spending any time I might have saved in the office), filled them, and brought them to the office. Boy, were they impressed! The forms were legible (much more legible than my poor handwriting). They actually hired me to make their forms fillable so they could post them. I did and they did. They are very happy with the results.
I also searched for patient information forms and downloaded 35. Guess how many were fillable? Only one, although two others did have a couple of fillable fields, but not all (what is up with that?). Imagine the time and money that could be saved with fillable forms!
Two recent conversations address another issue, filling out multiple forms with the same information, particularly if a hospital trip is involved. Wouldn’t it be nice to have generic healthcare forms that could be saved and printed on demand? I don’t think we are likely to see that…ever, given the current state of healthcare in this country. But trying to do my bit, I’ve created a two page patient info and patient history form as an example. Just for the sake of it, I made a few fields required and it’s submit-able to me (if those required fields are filled out). I’ve added a couple of explanatory notes along with a digital signature. And I’ve secured the form as it’s only a sample. Interested readers and any doctors can contact me for more information.
